Community Collaboration For Children

To meet the safety, permanency, and well-being outcomes in the Adoption & Safe Families Act (ASFA) of 1997, Title IV, as most recently amended.

 

Community Collaboration for Children (CCC) delivers quality services to prevent the abuse and neglect of children prior to involvement with the Cabinet for Health & Family Services (CHFS) or to prevent re-abuse and neglect by providing families with the assistance necessary to prevent the removal of children from the home and address the children’s physical, mental, emotional and educational needs.

LCCAA utilizes the services of a Licensed Practical Nurse and a Licensed Clinical Social Worker with a PhD. designation who is also a Certified Alcohol and Drug Counselor to provide intensive in-home services to families who are at risk of having their children removed from the home.

LCCAA also provides Supervised Visitation services for CHFS referrals.  LCCAA schedules visits and arranges/provides meeting locations in conjunction with CHFS and provides direct supervision of family visitations.  LCCAA also provides mentoring and coaching opportunities for parents in areas such as behavior, communication, and child development during visits/interactions with children. LCCAA provides documentation to CHFS of observations made during visitation

 

 

Eligibility  

For Intensive in-home services referrals can be accepted from any community partner. For Supervised Visitation services clients must be referred from CHFS.

 

 

Funding is in whole or in part from Federal. CHFS, or other State Funds.
